Type in the Certificate Number and Authorization Code that are printed on your certificate.

What should I do if I lost my certificate?
Please contact your local American Diabetes Association at 1-888-DIABETES.

When will I receive my gift selection?
Your selection will be shipped in approximately 15 working days after receipt of your order. Please allow up to 20 working days from the time you place your order until its arrival.

How do I know you received my gift selection order?
An e-mail confirmation of your gift will be sent to the e-mail address you provide. When your gift selection ships you will receive another e-mail update with the UPS tracking number.

When does my gift certificate expire?
Your certificate will expire 90 days after the date of issuance. The issue date and the expiration date are printed on your certificate.

I received a gift similar to but not the same as what I selected?
In certain cases, we will need to substitute a similar item of greater or equal value. We strive to work with manufacturers and offer products that will be available long term, but cannot always guarantee the duration of availability.

I would like to exchange my gift for a different selection.
The ADA does not allow the exchange of a gift selection.

I received my gift selection and it arrived broken or does not work.
Keep all original packing materials and contact Summit Marketing within 5 business days.
